Absences
Your child's absence needs to be recorded by an explanation from a parent/caregiver. Please see the form below to give to your child's teacher when he/she returns to school.
Medication
If you require the office staff at Kororo School to administer medication, you will need to fill out a permission note. It is very important for you to write clearly the type (name) of the medication and the correct dosage that should be given.
Note: Unfortunately in such a large school and with so many students requiring medication, it is up to your child to remember to go to the office for their medication. It may be an idea to put a reminder note in your child's lunch box.
Non prescribed medication
For any medication not prescribed by a doctor e.g.cough medicine, paracetamol etc that you can buy over the counter use the form request for administering non prescribed medication (PDF 110KB)
Prescribed Medication
For any medication prescribed by a doctor e.g. Ritalin, Ventolin, Epipens, Antibiotic etc use the form request for administering prescribed medication (PDF 47KB)
