If you require the office staff at Kororo School to administer medication, you will need to fill out the attached permission note. It is very important for you to write clearly the type (name) of the medication and the correct dosage that should be given.
Note: Unfortunately in such a large school and with so many students requiring medication, it is up to your child to remember to go to the office for their medication. It may be an idea to put a reminder note in your child's lunch box.
There are 2 forms attached, they are as follows:
Request for Administering NON Prescribed Medication (Non Prescribed Medication)
- this is for any medication
not prescribed by a doctor e.g.cough medicine, paracetamol.
Request for Administering PRESCRIBED Medication (Prescribed Medication)
- this if for any medication
prescribed by a doctor e.g. Ritalin, Ventolin, Epipens, Antibiotic etc
